黄大仙本乃金龟托世，千多年前，因不忍凡间受苦，擅作拯救，被黄母娘娘贬下凡间。大仙投胎农户之家，名黄初平，替刘家管羊。平与侍婢小兰感情要好，却被少爷针对，观音暗助。观音向平报梦，平遂向赤松观大师求道，获赠经修练。一别数年，平修为大进。大师临终前指点平日后去向，平决独自云游四海以救苦难，与兰诀别。平以大仙身份到处赠医施药，声名远播。王小二好赌成性，欲将妻子卖到妓寨，得大仙化解。天将不满大仙妄自施法，得观音平息。村民受毒水所困，大仙发现玄通道士竟假扮自己以敛财，二人斗法，玄通不敌，村民更感谢大仙功德。大仙故乡遭逢旱灾，大仙与村民一起求雨不果，原来此乃上天旨意，大仙向仙人求教，即以自身之血化雨挽救村民，终能成仙。
